Edward M. Kennedy Community Health Center, Inc. Telephone Triage Nurse in Worcester, Massachusetts

This position is 4 hours per week.

Job Summary:

Within the nursing scope of practice, the Triage Nurse provides guidance to patients and their caregivers via the telephone. The triage nurse is able to competently assess and advise the patient using the nursing process and following approved algorithms and protocols. The triage nurse receives patient calls from the call center, determines the urgency of the patient concern, determines appropriate levels of care needed, and provides symptom management and self-care advice. Telephone triage includes the care of neonates, pediatrics, young adults, and older adults throughout the life cycle.

Essential Functions:

  • Reviews and prioritizes organization-wide tasks and prioritizes and triages calls from patients requesting urgent care appointments when the call center schedulers are unable to provide urgent care and or same day appointments using organizationally approved protocols, policies and procedures, practice-specific instructions, and nursing judgment

  • Conducts sound nursing assessments and assists callers with issues related to the patient?s care utilizing active listening skills in order to receive detailed information in an accurate manner

  • Communicates with the care team effectively. Consults with appropriate care team members as needed. Refers triage tasks to clinical teams as necessary

  • Document patient care in the EHR. Maintains and updates EPM as needed.

  • Meets compliance and regulatory standards

  • Participate in practice- and organization-wide training and quality improvement initiatives

  • Performs other duties as assigned Education and Experience:

  • Massachusetts nursing license

  • Bachelor of Science in Nursing from an accredited college or university

  • CPR certified
