Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Float Registered Nurse in Worcester and occasional shifts in Leominster, MA.
Community Healthlink is a multi-service, private, non-profit organization committed to promoting, maintaining and restoring the dignity, well-being and mental health of individuals and families in Central Massachusetts.
The Float Registered Nurse will float among different programs as needed demonstrating the basic, functional knowledge of nursing skills, as a fundamental understanding of addiction and mental health issues in all client populations.

Assists in the writing of treatment plans, progress notes, evaluations and discharge paperwork in a clear and concise manner, resulting in completion of all assigned documentation daily. Provides accurate shift report to oncoming care team members.
Ability to develop good working rapport with clients, managers, and fellow colleagues. Participates as an inter-disciplinary team member. Attends treatment team meetings as needed.
Maintains highest standards of ethics and demonstrates professional attitude and conduct, while maintaining professional boundaries. Collaborates in a professional manner with other departments and programs within the agency as well as external providers.
Stays current and abides by all the standards of nursing practice as recommended by the American Nurses Association. Performs all job-related duties associated with each client care area as assigned by nurse manager or program director ensuring all agency policies and procedures are followed.
Renders and supervises care of the clients on the unit assigned.
Ensures safety of clients is always maintained. Should an incident regarding client safety or wellbeing occur an incident report should be completed at the time of the occurrence and submitted to the program supervisor for their review and input.
Determines and implements unit specific protocols as designated by the medical director or licensed prescriber.
Accurately transcribes, administers and documents all medications ordered by the licensed prescriber.
Ensures all necessary medications are ordered for discharge planning.
Provides patient education including but not limited to medication regimen, harm reduction strategies or any knowledge deficit areas identified by the care team.
Facilitates weekly nursing groups as needed.
Participates in multidisciplinary team meetings to discuss the client's status, progress, current issues and discharge planning. Assists in the coordination of additional consultation as needed, i.e. psychiatric referral.
Observes and assesses clients for medical/clinical signs and symptoms and follows prescribed course of treatment, with notification of acute changes to medical director or licensed prescriber.
Records all pertinent information related to the client's condition and the outcomes of treatment in the electronic health record.
Responds and provides emergency care to clients as needed (i.e. CPR, opioid overdose response, etc.) Activates EMS by calling 911. Report provided to the local ED which is receiving the client.
Notifies the provider, and unit manager regarding all medical emergencies. If emergency occurs after hours, contact the on-call manager.
Care areas which have inpatient referrals/admissions which require review the manager of the unit should be consulted. If the referral is received after hours and managerial input is needed, the on-call manager should be contacted for consultation.
Assists with the delegation of client related tasks to other members of the multidisciplinary care team as needed.
Participates in regularly scheduled meetings as requested by the nurse managers, attends/completes all mandatory assigned in-services. Participates in required continuing education modules to maintain current licensure and stay abreast of best clinical nursing practice.
Performs job related duties as assigned by nurse managers, program directors or other managers.
Assists with administrative tasks on the unit as needed, such as phone, door and deck coverage.
Performs controlled substance medication counts as needed by the unit being supported.
Must be able to bend, kneel and stand for long periods of time in order to assist clients when needed and to complete treatment protocols.
Travel between sites may be needed based upon unit staffing needs.
Provides culturally and linguistically appropriate care to all clients.
Assists with phone/fax referral process, reviews them for level of care appropriateness. In care areas where authorization is needed, assists with attainment of insurance authorizations. Adheres to specific program guidelines regarding the referral process.

Valid RN Massachusetts Nursing license.
2-3 years nursing experience with mental health and or substance use disorders preferred.
Appreciation and a desire to be part of an Integrated Care team which provides clients with optimal therapeutic care.
Understanding of trauma informed care and utilization of motivational interviewing strategies and techniques preferred.
Bilingual English/ Spanish helpful
Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes including travel between work sites
Must be able to pass a CORI
