Community Healthlink Registered Nurse 2nd shift Respite in Worcester, Massachusetts

Community Healthlink (CHL) is looking for full-time (37.5 hrs/wk) Registered Nurse working 2nd shift at our Worcester site.

Worcester Respite is a 3-bed DMH referred program. Respite clients reside on the unit for a set amount of time agreed upon by the treatment team and work towards their individualized treatment goals. Goals often include stabilization, housing, employment, and continued sobriety. Treatment team meetings are held with community providers, family and support systems, and Respite supervisors to assist individuals in meeting their treatment goals. Clients on the Respite program are encouraged to attend day programs, appointments in the community and to maintain daily structure.

The Nurse will be providing nursing services to consumers.

  • Provide nursing services to consumers.

  • Complete medical assessments for consumers.

  • Lead educational oriented groups about medications, medical needs, health and nutrition, and diagnoses.

  • Develop and implement trainings for staff and clients on medication, proper documentation, and other medically related matters.

  • Consult and collaborate with primary care physician and/or psychiatrist as indicated.

  • Perform physical assessments, document and monitor mental status and document changes, and contact proper medical personnel as status warrants.

  • Collaborate with other disciplines in aftercare planning for consumers.

  • Assist Program Coordinator in establishing and reviewing policies for standards of care.

  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

  • Perform other related job duties

  • Grade 9B: Acute Care RN: Graduate of an accredited school of Nursing with current license as a Registered Nurse in the state of Massachusetts.

  • Previous clinical experience working with substance abuse and mental health required.

  • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives required.

  • All bilingual candidates are strongly encouraged to apply.

  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle

  • Must be able to pass a CORI background check
