Edward M. Kennedy Community Health Center, Inc. Population Health Outreach Coordinator in Worcester, Massachusetts
The Population Health Outreach Coordinator conducts outreach activities with patients who are identified through quality and risk coding analysis. This employee utilizes reports to coordinate outreach activities to meet both internal and external requirements. The Population Health Outreach Coordinator coordinates the management of insurance membership lists by identifying and reaching out to members who are assigned to Kennedy CHC in need of PCP assignment or care.
Conduct outreach to patients identified as having care gaps, coding opportunities or need to be seen by provider and schedule appointments as needed. Document results of all contact attempts and dispositions of contacts.
Work on campaigns and initiatives which use data to improve complete coding, quality measure performance, key performance indicators and other priority areas.
Use technology to match member lists to patients in the Kennedy CHC electronic health record (EHR).
Follow defined protocols and scripts to reach out to members assigned to Kennedy CHC who are not current active patients of the health center or are active patients in need of services.
Implement workflows based on the member’s responses to questions about their current Primary Care Provider (PCP) situation. When the patient wishes to connect with our practice, follow designated provider assignment and appointment scheduling practices. For patients who have other PCPs or are not interested in care at the health center, assist patient to choose another PCP/ACO if desired.
Work with Population Health/Clinical quality team to improve priority clinical quality measures.
Manage various letters, mailings and other outreach communications
Ensure that all services are offered in a culturally and linguistically appropriate manner.
Ensure that all needed accommodations are consistently made for patients with disabilities.
Perform other duties as assigned by supervisor.
Required Education and Experience:
High School Diploma or equivalent
Strong attention to detail
Ability to learn an electronic medical record or other databases (Arcadia, Azara/DRVS)
Excellent communication skills (oral and written)
Proficient in Microsoft Office (Excel and Word)
Demonstrated knowledge of or experience working with the target communities
Reliable home internet service
Bilingual in English and other languages identified by the health center as being languages-of-need (such as Spanish/Portuguese)
Experience with Electronic Patient Management (EMP) and Electronic Health Record (EHR)
Certified Application Counselor (CAC)
Medical Assistant Certification
Flexible work schedule – may need to work evenings/occasional weekends to provide enrollee access and/or follow up
