Description and Responsibilities
The Program of Assertive Community Treatment (PACT) Team is a multidisciplinary team that provides an alternative to traditional outpatient treatment to any person who needs it within the community. The team provides 24/7 care and continuity of care (open-ended, lifelong) by mimicking hospital-level wraparound support to help individuals maintain in the community rather than bouncing back and forth from the hospital. We bring the support to our individuals by meeting individuals where they’re at, in settings and spaces where they feel safe and comfortable. Our unique contract with the Columbia Center for Practice Innovations provides the program and it's team robust, job-specific training.
The Peer Specialist is responsible for cultivating mutual, non-directive and supportive relationships that promote empowerment and self-learning to each individual served while maintaining appropriate boundaries. The peer specialist also facilitates peer skills building support groups and serve as role models to educated recipients about self-help techniques and self-help group processes.
Other Key Responsibilities Include:
Share personal experiences with the purpose and intent to build trust and collaboration with people served.
Assist peers in identifying and using strengths and interests to develop, implement, and monitor interventions and personal recovery goals.
Teach effective coping strategies based on personal experience, teach symptom management skills, assist in clarifying rehabilitation and recovery goals.
Assist in the development of community support systems and networks.
Qualifications
High School Diploma GED or equivalent, required.
Lived experience as a consumer/utilizer of the mental health system, required.
Valid Driver’s License, acceptable driving record, and reliable transportation, required.
Experience working with adults experiencing homelessness, mental illness and/or co-occurring mental health and substance use disorders, required.
Peer Specialist Certification, highly preferred.
