The MENTOR Network In-Home Companion in Worcester, Massachusetts
MENTOR Adult Day Health, a partner of The MENTOR Network, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
In-Home Companion We are seeking motivated and caring individuals to provide in-home support to participants receiving our services, including personal care and life skill activities. Part time and full time opportunities available.
Responsibilities may include but are not limited to:
Collaborate with the primary nurse and health care team members in implementing the plan of care to meet the participant’s goals
Responsible for the participant safety and well-being at all times
Interact with participants in an empathetic, respectful, caring, and dignified manner
Coordinate, organize, and/or assist with household activities
Maintain current progress and service notes and submits all required documentation and verbal feedback to Supervisor in accordance with the Mentor Network policy, program and Area Authority standards, and State Division of Mental Health/Developmental Disability/Substance Abuse policies
Provide emergency coverage and/or care for individuals receiving services
Qualifications:
High school diploma or GED
Minimum one year experience in providing services to individuals with intellectual and developmental disabilities, preferably with older adults
Must be minimum of 18 years of age
Home Health Aide or CNA experience preferred
Bilingual preferred
