Seven Hills Foundation Acute Care Clinical Admissions Case Manager in Worcester, Massachusetts

Exciting, high-paced role in Acute Care Services at the Wetzel Center. This position is responsible for supporting the acute care admissions and clinical departments by responding to, screening, and processing all referrals and referral inquiries from statewide emergency rooms and/or mobile crisis teams in a timely manner. Once admitted, this position will follow youth through their care by providing case management services. This involves developing a relationship with the youth and their family and working closely with their Wetzel Center clinician and psychiatry providers and their external care team. This position may be required to work in the milieu as a way to understand whether referrals will be an appropriate match for care at the Wetzel Center. This position requires a strong understanding of the youth emergency mental health and acute care services in Massachusetts as well as a strong understanding of youth and family serving systems.

  • Process acute care evaluations from external referral sources, schedule admissions and complete intakes, data management, coordinate with referral sources and insurance organizations, and manage multiple waitlists for placement.

  • Present evaluations to the Directors within the program for review and approval of potential admissions.

  • Coordinate with Clinicians and Clients/Families regarding necessary referrals, complete internal and external referrals and document referrals appropriately.

  • Develop a working knowledge of insurance driven services and community resources for discharge referrals.

  • Maintain positive and collaborative relationships with referral sources; maintain confidentiality, HIPAA regulations, and manage sensitive information.

  • Perform necessary data processing functions related to the program.

  • General duties include secretarial tasks, such as data entry, phone calls, typing, filing, and scheduling.

  • Attend all supervisions and meetings as assigned.

  • Experience with youth is required.

  • Experience with acute care programming is preferred.

  • Bilingual in English/Spanish preferred.

  • Ability to work effectively and efficiently in a high-paced, acute-care environment.

  • Strong customer service, oral and written communication, time management and organizational skills.

  • The ability to work collaboratively with large teams.

  • Excellent organizational skills, strong attention to detail, and timeliness.

  • Excellent computer skills in Outlook, Calendar, Word, Excel, PowerPoint, and Internet.

  • Valid driver's license and reliable transportation to fulfill essential job functions is required.

  • Knowledge of mental health, educational, developmental care, child welfare, and/or juvenile justice systems preferable.


  • High School diploma or equivalent is required.

  • BA in Social Work, Psychology, or Human Services is preferred.
