Position: Triage Nurse

Reports to: Clinical Programs Director

Job Description/Requirements:

As a Triage Nurse, you will:

  • Complete patient assessment via phone with experience clinical judgement considering the patient’s trends, history including, but not limited to documentation in the medical record and the patient or caregiver’s verbal description of the problem or condition.

  • Triage the call based on acuity or type of condition. Consider the type of patient/services received – Home Health or Hospice in determining the priority of the call.

  • Respond promptly to patient/caregiver calls and clinician requested tasks per phone and assignment of appropriate service codes in Point of Care database.

  • Review Care Plan to make best clinical judgement using critical thinking to determine intervention with problem (s) with education via phone, clinician visit or ER/ED visit.

  • Inform the physician, when indicated of the results of the assessment, labs etc. with possible HH/Hospice interventions based on assessment.

  • Re-evaluate the patients nursing, and other clinical needs as required conducting f/u calls as necessary.

  • Implement appropriate nursing interventions consistent with the patient’s diagnosis, established goals as indicated on the POC documenting interventions accurately/concisely. Maintaining accuracy in documentation is of high importance to convey actions taken and patient’s status

  • Complete workflow tasks accurately to expedite assignment of visits to meet the patient needs.

  • Document any new orders received from physician’s office, contact patient and notify of any changes and schedule any additional visits if needed to ensure compliance of teaching if needed. Write clear concise order as necessary

  • Practice within RN scope of practice according to designated Board of Nursing and support clinicians as necessary with policies and procedures.

  • Be accountable for own conduct and promotes good working relationships among nursing staff, interdisciplinary staff, and physicians.

  • Assign service codes to meet the need of the patient facilitating coordination of the patient care needs.

  • Perform in whatever capacity patient condition, census and staffing level requires including rotating work schedule with other RNs i.e. weekend, on-call and holiday work through consideration of visit capacity afterhours through collaboration with the Administrative staff on-call

  • Initiate professional growth opportunities by participating and attending educational programs and reading professional journals.

  • Practice nursing in accordance with the ANA Code for Nurses, Nurse Practice Act and the Patients’ Bill of Rights.

  • Project a positive image to other professionals, patients and public.

  • Maintain current knowledge of nursing, rehabilitative and preventive practices and theories through continuing education and in-services.

  • Perform work in a safe manner and complies with departmental and agency safety policies.

  • Participate in continuous Quality Improvement/Quality Assessment and Performance Improvement program as indicated.

  • Communicate frequently and works closely with other home care team members.

  • Participate in in-service training programs completing assignments timely based on due dates.

  • Perform other duties as assigned.

  • Maintain effective communication with patients, caregivers, referral sources, field (on-call clinicians), office staff, Admission Services/intake and Administrators on-call.

  • Make sure all written communications are clear and concise as documented in on-call coordination notes and/or Clinical notes.

  • Exhibit core values of respect, attitude of service and honesty and integrity in all aspects of role with team members and supervisor.

  • Communicate verbally and via email with Clinical Supervisors, Case Manager or Executive Directors with urgent clinical issues to ensure the patient’s needs are addressed and/or met.

Requirements of Qualified Candidates:

  • Must possess a current unrestricted RN license in state of agency operation

  • Preferred minimum of 5 years of experience as an RN and 1 in Home Health/Hospice

  • Sufficient professional verbal and nonverbal communication skills to perform required tasks

  • Must possess and maintain valid CPR certification while employed in a clinical role

  • Sufficient endurance to perform tasks over long periods of work hours

  • Ability to travel to all business locations

  • Willing and able to work remote Friday 4:30pm until Monday 8am.
