Description:

Mercy LIFE of Massachusetts, a member of Trinity Health PACE, is seeking a Home Care Registered Nurse for its location in W. Springfield. Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.

The MDS Coordinator oversees all aspects of Medicare requirements and MDS coordination. Is responsible for ensuring that the MDS accurately reflects the services provided. This person should be able to work with others to coordinate communication among all clinical staff.

PRIMARY JOB RESPONSIBILITIES AND DUTIES:

  1. RN, MDS EXP

  2. Responsible for required documentation for Medicare residents (admission through discharge).

  3. Monitors MDS data and other documentation for accuracy.

  4. Acts as a MDS/PPS resource to the interdisciplinary team.

  5. Performs electronic submission of the facility-wide MDS information to the state of Massachusetts.

  6. Reports all unusual incidents to the appropriate person/department.

  7. Actively participates in resident care planning providing RAP reports to the nurse managers. Promotes and supports the philosophy, goals, and objectives of the nursing department.

  8. Keeps accurate and timely documentation reflecting residents’ care and condition in accordance with state and federal regulations.

  9. Participates in project teams, as requested. Is an active participant in the facility’s CQI Committee.

  10. Responds to other related duties, as assigned by the Director of Nursing.

  11. Prints and distribute Quality Indicator reports timely, monthly, and as directed.

  12. Enters and maintains residents’ ICD9 codes in database, coordinating with medical records for updating face sheets.

  13. Responsible for Medicare Denial letters upon admission and decline determination and notification to a non-skilled level of care.

  14. Responsible for coordinating the routine review of residents in a Medicare Part A stay to accurately determine skill level.

  15. Participates in admission screening process to determine Medicare Part A eligibility and other MDS considerations.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  1. Education – Graduate of an accredited school or college of nursing. Current licensure as a RN in the state of Mass.

  2. Experience – Recent clinical experience in long-term care facilities or comparable clinical experience.

  3. Equipment – Resident related and within the scope and license of nursing.

  4. Specialized Skills – Excellent assessment and team work skills. Computer skills recommended.

  5. Must be able to safely perform the essential job functions with or without reasonable accommodation.
