7-3 Supervisor Duties
Upon arrival check daily allocation sheet for staffing ; make adjustments to sheet to reflect any changes and be prepared to fill unit vacancy should staffing warrant or as assigned by DNS
Check appointment calendar in supervisor’s office (M-F) for residents’ schedules, times, places, and CNA assigned
Check white board in supervisor’s office for DNS updates.
Obtain report from off-going supervisor
Count E-box narcotics and sign accordingly
1 st rounds-Review report information with charge nurses identifying problems, change in condition, pharmacy issues, supply needs, ensure paperwork for appointments is complete- adding any appropriate labs or x-rays etc; preparation for discharges.
Complete RN assessment and SBAR for residents with change in condition,
Conduct a walking round head count.
Review APRN book for entries and place calls as needed to APRN or MD.
Ensure that charge nurses have prepared C.N.A. worksheet with I and O’s, weights, showers, appointments.
Review BM book, I and O book, ADL flow sheets,Q 15 minute sheets and three day bladder diary. Address issues with the charge nurse.
(Mon-Fri.) Bring to Am report new/readmission, discharge, acute hospital, transfer, A & I’s, charts, I and O books, supervisor’s fluid tracking form. Report to clinical team highlights from the past 24 hour period (or 72 hours after weekend). Include results from previous day’s appointments, x-rays, tests, etc.
From Am report and team discussion give clear instructions and information back to the charge nurses
Confer with the APRN/MD (upon his/her arrival) and review clinical concerns, change(s) in condition, labs needed, follow-up.
Check lab fax machine in supervisor’s office for lab results and highlight log as received.
Check pharmacy fax machine in supervisor’s office for prior authorizations and ordered too soon notification. Please note that x-ray, ultrasound, dialysis info will be sent to the conference room printer.
Update census sheet with admissions, discharges, MLOA, room changes.
Complete acute transfer log when resident is hospitalized; update status next shift.
Make rounds to ensure that residents are groomed, beds made, and environment is safe.
Monitor and assist in dining room for lunch. Ensure seating plan is followed ; C.N.A.’s are present and timely; Residents are returned to their room after lunch for care.
Ensure 24 hour report sheets are thorough and completed by charge nurses. Add information as needed.
Assist charge nurses with transcribing orders, as time allows
Demonstrate good customer service with all contacts.
Update DNS as acute changes occur (falls w/injury, acute hospital transfers, risk issues)
Give report to on-coming supervisor including outstanding labs, calls to APRN/MD, residents on LOA, new admission paperwork in process, pending admissions. Count narcotics
Additional daily du ties as needed
Complete Re/New admission packet and required paperwork. Re-visit every new admission the next day to ensure paperwork is completed, medication is available, equipment needs met etc.
