Eliot Community Human Services Social Services RN (Waltham & Brockton) in Waltham, Massachusetts
Eliot is seeking an experienced RN with outstanding direct care based skills, experience with mental health and nutrition. The nurse will be responsible for the oversight of MAP for our Intensive Group Home Programs in Brockton and Waltham.
Eliot’s Social Services Division includes short term community based assessment programs and group homes for children (birth to 12 years old) and adolescents (13 to 21 years old). The programs are contracted with the Department of Children and Families and provide a safe, structured and respectful environment for children and youth to thrive, change and grow by focusing on treatment, skill building and permanency.
Identify, screen, and evaluate performance of youth with high medical needs using standard measures.
Provide initial and ongoing screening and physicals to youth entering and discharging from care.
Collaborate with youths’ PCP to ensure effective delivery of existing and new medical concerns and care.
Support the Family Team with the development and follow up of treatment goals for the Individual Service Plan in the home. Ensure plans are developed in accordance with the requirements of the funding source, sound clinical practice, and in collaboration with the family.
Facilitate and oversee medication training (MAP), protocols and compliance.
Consult with and support the program relative to youth with high medical needs.
Schedule: 10 hours a week. Hours are flexible.
Experience with population: Advanced
Med Administration: Expert
- Associates or better in Nursing
