The AFC RN provides nursing case management for AFC Participants and Caregivers including development and evaluation of AFC Plan of Care, goal setting, training, advocacy, and referrals. Schedule for this full time position is flexible based upon families' accessibility. Primary Responsibilities: Conducts on-site and telehealth visits with each participant and caregiver in the qualified setting monthly or bi-monthly and as needed. Development, implementation, and ongoing review of the AFC plan of care. Training, evaluating, and supervising AFC caregivers in conjunction with the case manager. Completes nursing assessments and progress notes, ensuring the documentation is completed in a timely manner. Monthly review of AFC care logs. Submits a semi-annual health status report to the member's primary care physician and communicates with primary care physician as needed. Assists the participant in obtaining information and accessing other healthcare and community services. Reviews and documents the fire and safety procedures for the qualified setting. Participates in discharge planning and implementation. Participates in ISP and team meetings as needed, attend other meetings as required. Interfaces with other agencies to coordinate services. Attends all mandatory agency trainings. Demonstrates and implements knowledge of Communitas policies and standards as well as funding source regulations. Follows HIPAA, security regulations and/or guidelines and maintain confidentiality at all times. Other duties and special projects as assigned. Education and Experience: Must hold current Registered Nurse (RN) license from the Massachusetts Board of Registration in Nursing. Minimum two (2) years clinical experience working with people with disabilities. Skills, Knowledge and Abilities: Excellent documentation and communication skills required. Must be able to develop individual treatment goals and plans and to monitor, assess and evaluate such plans. Ability to develop working relationships with other agencies and professionals for the purpose of optimal family benefit. Valid driver?s license and transportation required. Must be able to communicate effectively in English. Physical Demands: Must have physical capabilities to lift up to 20 lbs. Must be able to stand/walk up to one hour uninterrupted. Follow Universal Precautions at all times and wear protective equipment, as necessary. Must have the ability to achieve physical access to community, i.e., community organizations, offices, private homes. Must be able to exchange information face to fact, over the telephone and in written form. Location includes office space, program areas, in vehicles and in various locations out in the community including individual family homes. This position must help to maintain a safe working environment as determined by Communitas policies
