State of Massachusetts Performance Improvement LPN in Tewksbury, Massachusetts
Tewksbury Hospital under the Department of Public Health (DPH), a 370 bed Joint Commission accredited hospital, is currently hiring for the position of Performance Improvement LPN. The hospital provides a broad treatment of care and comfort to adults with medical and/or mental illnesses.
The selected candidate will work in the Quality Management Department and will be utilizing clinical knowledge to serve as a support to the LTCH Coordinator by assisting in the data collection and data entry required for the CMS Long Term Quality Indicator Project. They will keep current with CMS guidance relative to the LTC Indicator Project, complete chart reviews for incidences of SREs, will participate in presentation at RCA’s and Mock Tracers, collect quality indicator data, assist with the incident report function as needed, and perform other related duties as assigned.
Please Note: This position is 37.5 hours per week, Day shift, Saturday and Sunday off.
_Duties and Responsibilities (these duties are a general summary and not all inclusive):_
Serves as support to the LTCH Coordinator by collecting data, via medical record audits, required for timely completion, and submission of indicator data as required by the CMS Long Term Care Quality Indicator Project for Medical and Psychiatric services.
Keeps current with all guidance issued by CMS and NHSN relative to Long Term Care Hospital quality reporting requirements.
Completion of chart review in preparation of clinical reports that include key facts relative to incidence of a Serious Reportable Event (SRE) to a Root Cause Analysis (RCA) team for Medical and Psychiatric Services.
Collection of performance quality indicator data as assigned to meet various needs of the Quality Management Department.
Participates in/Leads Performance Improvement teams initiatives as requested.
Participates in Mock Unit Tracers and prepares resultant reports.
Assists as needed in the incident review process.
Serves on Nursing and Hospital committees as requested.
Attends required educational seminars to maintain licensure, and other workshops and seminars to enhance own professional development. Attends required In-service Programs annually as required.
Compliance with all applicable state and federal laws including the Health Insurance Portability and Accountability ACT (HIPAA) regulations which govern the privacy and confidentiality of information about patients.
Understands and initiates rules and regulations which govern fire, bomb threat and disaster.
Performs related duties as required.
The ability to audit medical records and extrapolate clinical data
Data aggregation relative to SREs
Computer skills relative to inputting, analysis of data and presentation of information in reports and graphs.
Clinical working knowledge of CLABSIs, CAUTIs, and Pressure Ulcers.
Knowledge of external accrediting/regulatory agencies such as CMS, CDC, TJC, DMH and DPH
Extensive experience with Microsoft Word and Excel

MINIMUM ENTRANCE REQUIREMENTS
Applicants must have at least one year of paraprofessional experience in the field of nursing as a licensed practical nurse.
SPECIAL REQUIREMENTS: Possession of a current and valid license as a practical nurse under the Massachusetts Board of Registration in Nursing.
