State of Massachusetts Licensed Practical Nurse (LPN II) - Temporary 2nd shift in Taunton, Massachusetts

The Department of Mental Health (DMH) is seeking an exceptional Licensed Practical Nurse for this temporary full time position at the Southeast Area On Grounds Program to work as a direct service provider while maintaining a person centered, trauma informed atmosphere and professional performance. Knowledge of DMH community residential regulations and program procedures, coordination of all aspects of the program which includes but is not limited to: monitoring behavioral, psychiatric and health care needs and safety concerns, providing direct nursing and all aspects of care to clients, ensuring that client’s needs are met, and overseeing that Community Service Plans are being followed and appropriate documentation occurs, ensure that safe staffing is in place, follow-up of medical appointments, educating clients and staff about rehabilitation, medication and physical and psychiatric illnesses, coordination of physician’s orders, prescriptions and recording of all necessary paper work, notification to program director /assistant program director of pertinent information and performs related work as required. This position requires transportation of clients to appointments and other community activities. Please Note: This is a temporary position to cover an individual on a leave of absence. The position is 40 hours per week. The selected candidate will work on 2nd shift ( 2:45 pm to 11:15 pm). The days off are Friday and Saturday. _Duties and Responsibilities (These duties and responsibilities are not all inclusive):_ * Ensure care and rehabilitation is provided as prescribed by the Community Service Plans (CSP). Ensure safe staffing is in place and coordinate with the Administrator on Call (AOC), when necessary. Communicate client and program information amongst houses and staff to ensure a safe working environment. * Coordinates medication services which includes following through with physicians orders, prescriptions, filling out Residential Information Sheets and checking to ensure that each coincides, ensuring that medications are sufficient to last until the next appointment, writing self-medication check off sheets, and posting medication changes to ensure clients receive the appropriate medication regimen. * Monitors, reports and records clients response to current medication and treatment regimens and reports findings to the supervisor for follow up with appropriate medical practitioner. * Educates clients and staff about medications and their effects by utilizing in-service trainings, their own professional knowledge and resources located at the program to ensure that they have a basic knowledge and understanding of such. * Administers medications and treatment orders as prescribed by an appropriate medical practitioner. * Accompanies clients and ensures that clients receive emergency care, as needed. * Maintains and monitors a medication closet, first aid and infection control supplies to ensure that everything is labeled, kept in order, and that adequate supplies are on hand, and have not expired. _Required Qualifications:_ * Knowledge of the principles and practices of practical nursing. * Ability to effectively communicate, orally and in a written format, in English. * Ability to work as a member of a team and individually. * Knowledge of care of special client groups, such as psychiatrically impaired individuals. * Possession of a current and valid Massachusetts Class D Driver's License or equivalent required for this position. _Preferred Qualifications:__ _ * Experience working in a Community Mental Health Setting. _Working Conditions:_ * While performing the duties of this position, incumbents work under stressful situations and under time pressures. * Applicants may be exposed to verbal and/or physical confrontations and potentially dangerous environments. * Incumbents will work days, holidays, and may work extended hours. _DMH Mission Statement:_ On Grounds Program is part of the Taunton/Attleboro Site that transitions clients from an inpatient setting to a more independent community living setting.
