REPORTS TO: Regional Director Home Health
SUPERVISES: Clinical Team Members, RNs, LPNs, HHAs

**$15,000 Sign-On Bonus (based on FTE)
POSITION SUMMARY :
The Manager of Clinical Practice (RN) directs and supervises the office operation and will be available for OASIS Assessments in the field. Guides team in planning and providing individualized care and comprehensive services to home care patients. Assists in clinics and other agency programs as assigned by the Regional Director. The Manager of Clinical Practice (RN) is expected to meet established productivity standards as well as a rotation of manager on-call responsibilities.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :
Monitors assigned workflow tasks in HCHB
Provides clinical direction to field staff as needed
Codes commercial OASIS
Organizes educational programs to address identified needs
Reviews clinical records for completeness
Demonstrates prioritization, organization and appropriate management of time
Keeps current with all regulatory requirements affecting Home Health services
Oversees Medical Supply orders
Collaborate with supervisory staff to promote accuracy and effectiveness in documentation
Participate and lead case conferences and staff meetings
Educates staff on deficiencies found through audits as needed
Evaluate clinical competency of nursing staff
Participates in the orientation of new team members
Manage Telehealth programs
Performs Blood Pressure clinics , as requested.
Assists with clinical oversight at the Overlook IL and AFL as needed
Provides direct skilled nursing care as per physician orders in the field with patient visits as needed
On-Call triage coverage as needed.
Documents in medical record and prepares reports as needed.
Educates patient and families in self-care and health management
Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
Adheres to policies and procedures
Participates in agency quality management and safety activities
Serve as coverage role during staffing challenges
Any additional responsibilities as requested by the Regional Director Home Health and Executive Director Home Health and Hospice
SUPERVISORY RESPONSIBILITIES
Acts as Home Care Home Base (EMR) resource with extended access and roles
Completes RNs, LPNs and home health aide supervisions and orientations as needed
Participates in training new clinicians on HCHB and homecare regulations
Co-signs orders and provides oversight of care for RNs and LPNs
QUALIFICATIONS :
Graduate of an accredited school of nursing required, BSN preferred, MSN a plus or Bachelor’s degree in another discipline with home care experience
Minimum 3 years’ experience in a managerial role
Minimum 5 years’ experience in a patient care setting
Current license to practice as a Registered Nurse in the state of Massachusetts required
Current CPR required
Current MA Driver's License and reliable transportation required
Demonstrated collection and supervisory skills
Excellent organizational and interpersonal skills
Computer proficiency
Excellent verbal and written communication skills
Ability to read, write and speak the English language
Maintain required vaccinations
PHYSICAL REQUIREMENTS/WORKING CONDITIONS :
Primary work will be performed in doors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 25 pounds. Completing visits in the field as necessary requires driving to patient locations.
