POSITION SUMMARY :

The Admission Registered Nurse is responsible for conducting admission assessments with a focus on developing a patient specific plan of care. The Admission Registered Nurse in this role provides quality and safe delivery of nursing and home health services that meets the organization’s mission of providing exceptional patient care.

POSITION DUTIES/ESSENTIAL FUNCTIONS :

  • Assesses patient’s health problems and develop patient specific plan of care.

  • Provides physician ordered skilled nursing services.

  • Completes 2-3 admissions per day.

  • Assess appropriateness of patients at SOC to ensure they meet the following criteria that are justifiable for services: homebound, identifiable skilled need and focus of care.

  • Has a clear understanding of oasis and plan of care development.

  • Educates patients and families on patient specific plan of care.

  • Collaborates with the physician, home health team members and other care providers, within and outside agency to enhance patient care.

  • Attending case conference as requested.

  • Adheres to policies and procedures.

  • Participates in quarterly QAPI meetings.

  • Provide other administrative support as necessary, including scheduling group meetings, maintaining calendars, doing research, and creating reports.

               

    POSITION REQUIREMENTS:

  • Graduate of an accredited school of nursing required

  • Current license to practice as a registered nurse in the state of Massachusetts required

  • Current CPR required

  • One year nursing experience, preferably med-surg required

  • Current MA driver’s license and reliable transportation required

     

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS :

    Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 35 pounds.

The organization embraces its rich history and heritage, owning and managing 218 Independent Living Apartment Homes and Cottages, 28 Enhanced Care Private Suites, a 14- Suite Assisted Living Memory Care neighborhood, 27-unit Short-term Post-Acute Rehabilitation, 112-bed Long Term Care Skilled Nursing, as well as Home Health, Hospice, Palliative Care, Private Duty and Care Management divisions.

     

    The Overlook and its 3 subsidiaries are based in Charlton, MA, with satellite home health and hospice offices across the state. The organization employs approximately 600 talented and compassionate Team Members and serves 1,500+ Residents/Patients.
