Amergis RN Case Manager Openings -Western Ma in Sunderland, Massachusetts
Amergis Staffing is seeking a Nurse Case Manager (RN) to work in the community with adults and geriatric. The ideal candidate would possess knowledge of Case management, Hospice, Home Care, Skilled Visits. This person would be comfortable with outreach, case management and providing support in individual’s homes.
Details of the position include:
Location: Greenfield and Northampton, MA
Program: VNA
Schedule: Monday – Friday 8am – 5pm * Times flexible if needed,
1 weekday evening on call from 5pm – 8pm, and 1 weekend on call every 4 weeks
Start Date: Two weeks from offer
Salary: $50-55/hr
Duties and Responsibilities (these are a general summary and not all inclusive):
The Registered Nurse Case Manager plans and provides individualized nursing care and comprehensive services to home care patients.
Assesses patient’s health problems and needs and develops plan of care.
Provides direct skilled nursing care as per physician orders.
Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.
Document in medical record and prepares other reports as needed.
Educates patient and families in self-care and health management.
Qualifications
Must have 1 year of relevant experience ( Case manager, Hospice, Home Care, Skilled Visits)
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
