About Summit ElderCare :
Fallon Health operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England and the fifth largest in the country. Called Summit ElderCare, Fallon’s PACE helps provide older adults and their caregivers with a welcome alternative to nursing home care. Participants in Summit ElderCare have access to comprehensive medical services and social support at a Summit ElderCare site while they keep the independence of living in their own homes and communities.
Full time position, responsible for developing and implementing a therapeutic activity program which meets the needs of the participants in conjunction with PACE center staff and restorative consultants (P.T. O.T., S.T.).
Utilizing input from restorative service consults, develops and implements a plan of therapeutic activities for each participant to promote optimal health, independence and self care.
Assesses individual abilities, needs and preferences through an activities assessment during the intake process.
Identifies problems, establishes realistic goals and sets up treatment plans utilizing therapeutic activities.
Participates in the interdisciplinary team (IDT) process and collaborates with IDT members to meet the needs of participants.
Participates in daily IDT meetings, care planning and quality meetings as necessary.
Periodically reassesses participants’ needs and communicates with IDT members to update participants’ care plans as needed.
Documents quarterly review of participants’ needs and participation in activities in the participant’s medical record.
Plans, organizes and implements, or supervises the implementation by others, the daily schedule of individual and group activities tailored to the needs of frail elderly participants.
Schedules, posts and distributes the monthly activity schedule/calendar.
Supervises Activity Assistants in the provision of their responsibilities.
Develops the volunteer program including recruiting, orientation and supervision. Develops and maintains the volunteer handbook and records accurate volunteer data and statistics.
Orders supplies and equipment needed for activities. Keeps a written record of expenditures.
Participates in the recruitment and hiring of Activity Assistants and makes recommendations to the Site Director.
Maintains accurate census records of daily attendance at the PACE center.
Assists with other PACE center services as assigned.
Facilitator for monthly activities meeting
Review and have input into monthly activities programming for all pace sites.
Will act as subject matter expert in all areas of Recreational Therapy Programming
Performs all duties in accordance with FCHP and Summit ElderCare policies and procedures.
Valid Driver’s License and Clean Driving Record Required
High school graduate. Associate or Bachelor’s degree preferred. * *
Certification as an Activity Director preferred.CPR and Alzheimer’s certification, or willingness to be certified, is essential.
Certification in Recreational Therapy
At least 2 years of experience working in a social or recreational program providing and coordinating services for a frail or elderly population within the past 5 years in a healthcare setting.
