At Mercy Medical Center the Inpatient Care Coordinator you would play a crucial role in facilitating patient care within Mercy Medical Center. What you will do:

  • Patient Assessment : Conducting initial assessments of patients to determine their healthcare needs, including physical, mental, and emotional conditions.

  • Care Planning : Developing individualized care plans in collaboration with the healthcare team to address patients' needs and goals.

  • Coordination : Serving as a liaison between patients, families, healthcare providers, and other stakeholders to ensure seamless communication and coordination of care.

  • Resource Management : Optimizing the use of resources, including medical equipment, personnel, and facilities, to provide efficient and effective patient care.

  • Patient Education : Providing patients and their families with information about their conditions, treatments, and self-care techniques to promote health and prevent complications.

  • Advocacy : Advocating for patients' rights and preferences within the healthcare system to ensure they receive the best possible care.

  • Documentation : Maintaining accurate and up-to-date records of patient assessments, care plans, interventions, and outcomes in compliance with regulatory standards.

  • Quality Improvement : Participating in quality improvement initiatives aimed at enhancing patient outcomes, safety, and satisfaction.

Minimum Qualifications

  • Education : Graduate of an accredited college or university affiliated nursing program. BSN graduate preferred.

  • Licensure : Current Licensure as a Registered Nurse RN in Massachusetts.

  • Certification : BLS (AHA/ARC). CCM or ACM Certification preferred.

Preferred Skills:

  • Experience: 3-5 years in Case Management, Utilization review, or similar experience preferred. Hospital or community-based experience preferred.

Work Schedule : Full time – 40 hours – Days
