Baystate Health Patient Flow Coordinator RN Registered Nurse in Springfield, Massachusetts

The Patient Flow Coordinator is responsible for clinical and non-clinical patient flow and throughput, utilizing leadership, project management, data analysis, and process improvement skills. The Patient Flow Coordinator is accountable for assessing, planning, directing, and evaluating throughput and patient flow activities (admissions, discharges, transfers, post-operative patients, etc.) in partnership with PPM and units to expedite efficient bed placement. The coordinator works with members of the administration, physicians, and other members of the multi-disciplinary staff to ensure that appropriate patient placement and movement are timely and efficient. The coordinator is accountable for ensuring that patient transfer occurs within a timely and efficient timeframe set forth by established national benchmarks and ensures processes for quality patient care and problem resolution are in place as it relates to patient flow and throughput.

Job Requirements:

1)Collaborates with PPM to facilitate inpatient admissions and transfers

2) Facilitates and leads bed huddles with key stakeholders such as PPM, CSO, Charge Nurses, Nocturnists, and appropriate ancillary departments

3) Collaborates with physicians and other providers regarding patient flow issues

4) Coordinates multi-disciplinary efforts to facilitate Throughput & Patient Flow while engaging ancillary departments as necessary

5) Ensures optimal utilization of resources (including but not limited to staff, equipment, and supplies) and implements resources according to need

6) Communicate with off-going/on-coming Nursing Supervisors and CSOs to ensure appropriate staffing for acuity and census

7) Frequent communication with ED, Peri-Operative Services, and Patient Access, maintaining constant understanding of bed needs/availability

8) Exercises judgment to reach decisions, considering individual needs and the good of the organization. Defends sound decisions with facts regardless of opposition and pressure. Works toward consensus whenever possible

9) Anticipates Throughput needs (i.e., review of surgical schedule, direct admissions, ED census, discharges, transfers, etc.)

10) Apprises nursing leadership of anticipated throughput obstacles, developing issues, and short-term/long-term action plans as appropriate

11) Collects, analyzes, and reports throughput metrics to relevant stakeholders

12) Proficient in utilizing technology, tools, and systems to manage patient flow (i.e., Teletracking, CIS, FirstNet, data collection, management systems, etc.)

13) Reports to Incident Command when activated to manage/facilitate patient flow

14) Responds quickly and efficiently in crisis situations

15) Delegates to staff dependent on needs of patient flow initiatives

16) Maintains evidence-based practice regarding the safety, patient experience, and overall importance of patient flow and throughput

17) Effectively adapts and adjusts to changing conditions, stressful situations, and the daily pressure of a dynamic system. Assists staff in adjusting, clarifies the rationale for change, and providers information and a forum in which to process events

QUALIFICATIONS:

  • Successfully passed Board of Registration in Nursing Examination and is licensed in the Commonwealth of Massachusetts

  • Current American Heart Association Health Care Provider CPR certification required.

  • 5 years of inpatient nursing practice

  • Education: ADN required; BSN preferred

SCHEDULE:

  • Evening/Night (3) 8 hour shifts 24 hours per week- 4PM-12:30AM every other weekend every other holiday

  • Location: Baystate Medical Center Springfield MA

Must possess well-developed critical thinking abilities and exercise sound clinical judgment. Must be recognized by peers as a leader and resource person who is sought out for problem-solving capabilities. Must have good organizational and communication skills.

The base pay range shown above considers the wide range of factors that are considered in making compensation decisions including knowledge/skills; relevant experience and training; education/certifications/licensure; and other business and organizational factors. This base pay range does not include our comprehensive benefits package and any incentive payments that may be applicable to this role.
