Gandara Center Nurse in Springfield, Massachusetts

We have an exciting new opportunity for you! We're searching for a Nurse - (RN/LPN) to join to join our new Certified Community Behavioral Health Clinic (CCBHC) Initiative in our Outpatient and Recovery Services Division. The Nurse (RN/LPN) will be working the Medical Director and will provide direct and individualized nursing care, utilizing proven wellness and health principles, to assigned groups and clients. The candidate we are looking for will respond to the needs of clients including addressing symptoms commonly associated with Substance Use Disorder [SUD] and mental health needs, implementing proper protocols, communicating with clients' primary care physicians, and providing education and training to project staff on relevant medical/health topics. Essential Functions: Vital measurements: respirations, heart rate, temperature (P.O., tympanic, and axillary routes), pain assessment: lung sounds, heart sounds, neurological, gastrointestinal, signs of abuse/neglect, injuries, seizure precautions and crisis management. Nurse will collaborate with the individuals and their families, educating them and recognizing the unique needs of the dually diagnosed and other special populations to obtain positive therapeutic outcomes. Evaluate the individuals' responses to the care plan and track their progress towards ideal outcomes Monitor medication as prescribed by the prescriber and in compliance with applicable regulations. May be required to obtain laboratory specimens (including phlebotomy), and complete other assigned tasks within the scope of practice as directed. Counseling; assumes counseling responsibilities in health-related areas, including but not limited to: prenatal care, child and nutritional care, and general hygiene. Create and provided educational groups base on wellness: Like nutrition, diabetes education, pain relief education, healthy exercise habits. Reporting; reports changes in condition to the provider and/or clinic supervisor, record keeping and documentation; documents clearly and completely services provided, the patient's response, and any change in the condition noted. BMI follow up and education Participate in weekly mutli-disciplinary team meeting Required Skills Minimum Qualifications Bachelor in Nursing, or Associate in Nursing, with experience and a MA license Certification as a registered nurse or nurse practitioner by the Board of Medicine, Nursing Division CPR, MAP training Bilingual [English & Spanish] encouraged to apply. Registered Nurses should have the following qualities: Proficiency in applying the nursing process to individuals and families. Ability to collaborate with other caregivers. Ability to delegate to peers and other caregivers consistent with their level of expertise. Ability to understand, empathize, and work with an addicted population, including those with co-occurring disorders. Knowledge of quality assurance standards. Working knowledge of ethics and legal parameters of working with substance abuse clients.
