MetroCare of Springfield is a leading Adult Foster Care and Group Adult Foster Care provider. Our nurses are an integral part of our care team, they are the bridge between a client\'s medical provider and caregiver. We are searching for a passionate LPN to coordinate and nursing schedules from within our downtown Springfield office. Eligibility LPN must be fully licensed by the Massachusetts Board of Registration. Must have experience with management of other clinicians and their schedules Ability to supervise and advise other clinicians 2 years of recent experience in the direct care of the geriatric population or adults with disabilities (Clinicals, nursing/medical assistant, internship experience accepted) Proficient computer literacy- Microsoft applications, navigating PDFs, etc. Job Responsibilities Conducting and coordinating all applicable clinical assessments and clinical evaluations. Visits must be completed in the qualified setting. To ensure the safety of our team, we have COVID 19 precautions in place for in-home visits and telehealth flexibility as needed (subject to change in the upcoming months- MassHealth regulations). Reviewing Staff Schedules to ensure visit compliance Completing documentation corresponding with each on-site visit or encounter. Examining each client?s AFC plan of care semi-annually. Selecting, training, evaluating, and managing AFC caregivers in conjunction with the case manager. Providing ongoing training for AFC caregivers. Monitoring the health status of all clients to ensure that they are receiving the best care possible and reporting any changes to the client\'s PCP. Reviewing and updating client medications and diagnoses. Planning for and implementing discharges and transition from the AFC program.
