Health Services Specialist 5 - Outreach RN Are you a nurse looking for a case management role that offers a flexible schedule, no required overtime, no mandated shifts, and no hands-on nursing? Then this is the position for you! **\$500 sign on bonus (\$250 at the completion of first week of training and \$250 after successful completion of first 90 days) ** Position: Case Management Nurse / Part Time Location: Springfield, MA Rate of Pay: Starting at \$34.00 per hour Hours: 20 Hours per week Shift: Monday-Friday; Day Shift flexible schedule Summary: The case management nurse relies on previous work experience and works under limited supervision, using knowledge of clinical practices to assist members with health and occupational or physical therapy activities. \"Retired case management nurses, recently graduated nurses, and nurses seeking flexible, day shift schedules are strongly encouraged to apply!!\" Duties and Responsibilities: Requires 1-2 home visits per day. Required home visits and meetings in person. Conducts clinical eligibility assessments for adult foster care members. Completes prior authorizations. Works with limited supervision Ensures programmatic and contractual obligations are met. Assists with program organization, program expansion, preparation and trainings of staff who will be implementing program activities in other areas. Qualifications: Must hold a MA RN license. At least 2 years of human services experience preferred. Must have a valid driver\'s license, good driving record, and a reliable vehicle. Knowledge of and respect for diverse populations. Excellent communication, interpersonal, teamwork, and listening skills. Demonstrated reliability and dependability.
