State of Massachusetts CO Nursing Facility Specialist in Springfield, Massachusetts

Mission Statement: The Department’s mission is to create, in partnership with others, innovative and genuine opportunities for individuals with intellectual and developmental disabilities to participate fully in their communities and meaningfully engage as valued members. _The Central Office Nursing Facility PASRR Specialist_is responsible for conducting Level II PASRR(Pre Admission Screening & Resident Reviews)evaluations for all individuals having or suspected of having an intellectual disability or related condition such as a developmental disability who are seeking admission or continued stay in a nursing facility, pursuant to federal law. Level II PASRR evaluations are conducted are in hospitals, nursing facilities, various community settings. The overall evaluation includes federally required elements such as review of medical/health needs, behavioral/mental health concerns or issues, and functional abilities with ADLS (Activities of Daily Living)/IADLS (Independent Activities of Daily Living). Additionally, the PASRR Specialist provides* direction to nursing facilities and local area office staff regarding concerns or issues for individuals that have been admitted to a nursing facility. Reviews current plans of care, community or medical resources, discharge planning, and transitional efforts which have presented as barriers to discharge. Provides direction and guidance regarding Masshealth & DDS regulations; and functions as a liaison with local ASAPS (Aging Service Agencies Point of Service) health care agencies, other state agency partners (Masshealth, Department of Public Health, and Massachusetts Rehabilitation Commission). Responsible for the oversight of discharge planning for all nursing facility residents with a PASRR assessment. Ensures that DDS Area Offices are familiar with departmental policy regarding community placement.. Ensure identification of placement by day 60 of the person’s nursing facility admission. Ensure that placement is successfully implemented through ongoing oversight and monitoring, close coordination, and technical support of Area Office placement efforts. MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) four years of full-time, or equivalent part-time, professional, administrative or managerial experience in business administration, business management or public administration the major duties of which involved program management, program administration, program coordination, program planning and/or program analysis, and (B) of which at least one year must have been in a supervisory capacity, or (C) any equivalent combination of the required experience and the substitutions below. Substitutions: I. A Bachelor's degree with a major in business administration, business management or public administration may be substituted for a maximum of two years of the required (A) experience.* II. A Graduate degree with a major in business administration, business management or public administration may be substituted for a maximum of three years of the required (A) experience.* III. A Bachelor's or higher degree with a major other than in business administration, business management or public administration may be substituted for a maximum of one year of the required (A) experience.* Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.
