Kennedy - Donovan Center, Inc. Physical Therapist in Southbridge, Massachusetts

As a member of a trans-disciplinary team and in partnership with families, promote optimal developmental skills through the provision of activities to meet outcomes outlined on Individualized Family Service plans (IFSP). Responsibilities: - Participates as a member of the trans-disciplinary team in program planning and delivery of services. - Participates in intake/screening of new clients - Participates in initial assessments and annual evaluations of children and families contributing to the development of IFSP - Maintains safety and medical standards according to agency policy and procedures - Maintains expected level of productivity - Provides case management and referral services to assigned caseload, including participation in interagency case conferences - Provides home visits to assigned caseload, for purposes of providing developmental stimulation, parent training and support - Responsible for implementation of IFSPs - Participates in parent/child and/or child groups - Provides co-treatments with other team members home and center-based - Provides parent education activities - Provides teaching and consultation to team members - Completes narrative reports of assessments, progress reports, IFSPs and discharge summaries - Maintains appropriate communication with physicians, clinics, and community agencies participating each child's care - Coordinates transition/termination process for clients, including referrals to Chapter 766 services when appropriate - Maintains assigned client records according to program procedures - Demonstrates the ability to work collaboratively in a team approach to service provision - Maintains compliance with all KennedyDonovanCenter policies and procedures, OCCS/DPH regulations and requirements. - Participates in professional development activities and training necessary to maintain relevant certification/licensure in accordance with their professional discipline and in accordance with EI Operational Standards - Participates in in-service training, team meetings, group and individual supervision. - Assist in outreach and fundraising activities. - Perform other job-related responsibilities as assigned. Qualifications: - Must have Physical Therapist License and requirements as defined in Early Intervention Operational Standards - Excellent organization, interpersonal and communication skills - Able to meet required competency level in Microsoft Office Software. Physical Requirements: - Frequent movement within groups including but not limited to kneeling, bending , lifting, pushing and pulling. - Ability to use a computer.
