Open Sky Community Services ACCS Team Leader - South County Integrated Team in Southbridge, Massachusetts

Description and Responsibilities
Adult Community Clinical Services (ACCS) is a Department of Mental Health (DMH) funded service that is designed to deliver evidence-based interventions within the context of a clinically focused model. Individuals served reside in the community, both in their own homes and in group living environments (GLEs). ACCS Teams are multidisciplinary, and include clinicians, substance abuse counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff.
Team Leaders provides leadership to the Integrated Team as well as direct supervision and support to Outreach Counselors, the Housing Coordinator, and the Administrative Assistant.
The Team Leader will:
Hire, supervise, mentor and evaluate direct care staff.
Develop and manage the program budget.
Ensure optimum coverage through the development and monitoring of the schedule.
Responsible for the implementation of high quality rehabilitation utilizing evidence based practices according to each person’s Treatment Plan, and with supervision from a Licensed Clinician.
Collaborate with Individuals Served, Licensed Clinician, Registered Nurse, and other members of the team in the implementation and monitoring of any self management and health and wellness plans and activities.
Facilitate and monitor the implementation of activities that create opportunities for the development of valued roles and personal relationships in the community.
Assist people to access, and/or provide, all needed transportation.
Participate in On-call Beeper rotation.
Qualifications
Master’s or Bachelor’s degree in Social Services preferred.
Five years of experience in direct care.
High School Diploma and valid driver’s license required.
