Next Step Health Care MDS Coordinator/RN in South Dennis, Massachusetts

South Dennis Healthcare is seeking an experienced MDS Coordinator for a full-time position. The responsibilities of the MDS Coordinator includes but is not limited to overseeing and coordinating of developments and completions of the member assessment ( MDS / PPS) in accordance with current Federal and state regulations.

Duties of the MDS/PPS Coordinator include but are not limited to:

  • Completes assessments, Minimum Data Set (MDS} and care plans for all residents assigned.

  • Monitors completion of MDS by other disciplines within timeframes prescribed by regulatory guidelines.

  • Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.

  • Initiates care plans and supporting activities that will result in best possible outcome for assigned residents.

  • Conducts Utilization Review meeting. Ensures that all disciplines have updated and accurate information.

  • Maintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues as necessary.

  • Reviews all resident incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.

Responsibilities and requirements for MDS / PPS Nurse Coordinator:

  • Valid RN License in the state of MA

  • Clinical experience, education or specialty skills specific to geriatrics is desired.

  • Great interpersonal skills for effective daily interaction with Residents, family members and all members of the Interdisciplinary Team

  • Must have knowledge in PPS assessment as well as OBRA compliance.

  • Must be a self-starter and a high attention to accuracy and details and possess good time management skills

  • Must have at least 1 year of MDS coordinator experience. Additional experience and performance achievements preferred.
