Essential Functions:

  • Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction and 90 day evaluations

  • Complete initial client assessment, plan of care, 90 day reassessments, and ongoing evaluations of the GAFC setting

  • Partake in both care plan creations and reviews, along with discharge plans

  • Assess and address motivational, psychosocial, and environmental concerns

  • Maintain and update client charts, medical records and submit all relevant information to authorized organizations

  • Completing notes corresponding with each on-site visit or encounter, or more often as the member's condition warrants

  • Document health changes and communicate to the client's Case Manager and Program Manager in a timely manner

  • Review all paperwork and plans of care to ensure program compliance, updating information as needed

  • Performs any other additional duties that may be required by management at any time

Education, Experience and Licensure Requirements

  • Licensed by the Massachusetts Board of Registration in Nursing

  • Must have at least two years of recent experience in the direct care of elders or adults with disabilities

  • Excellent interpersonal and communication skills required

  • Valid driver's license with proof of insurance required

  • Bilingual in English and Spanish strongly preferred

  • Relevant experience, preferably in healthcare, home health care, case management, or sales preferred

  • Previous experience with mental health aspects of care

  • Strong communication, writing, organizational, and time management skills

  • Solution-oriented with the ability to multitask

  • Proficiency with the Microsoft Office Suite
