Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation and hope to those in services. Currently the team is seeking an experienced, dedicated RN Nurse Manager. Eliot's Behavioral Health Community Partner (BH CP) program is an innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) redesign within Massachusetts.

Responsibilities:

  • Responsible for supervision of BH CP Nursing staff with best practices and strategies to meet identified paperwork targets, Quality Metrics goals, ACO/MCO initiatives, and Key Performance Indicators (KPI).

  • Responsible for training all new nursing hires and Nurse interns.

  • Review and/or disseminate for review all member Inter/RAIs for clinical accuracy

  • Act as a preceptor by providing coaching, education, and consultation to the entire BH CP program as well as within the Care Transitions Team (CTT).

  • Incorporate a community nurse treatment model that includes consultation and intervention of medical, behavioral and (SDoH) health care needs.

  • Participate in relevant care team meetings (internal and external) to ensure appropriate nursing involvement/intervention as it relates to member care.

  • Oversight and tracking of Transitions of Care metrics and workflows including Follow Up after Discharge (FUD) Medication Reconciliation, HEDIS and other initiatives.

  • As needed, provide side by side interventions to enrollees that focus on education and skills training in relation to health and wellness issues.

    Qualifications:

  • Licensed registered nurse in Massachusetts with at least a bachelor’s degree in nursing and minimum of 2 years’ experience working with adults with complex medical and behavioral health diagnoses.

  • Must possess knowledge of psychiatric medications, psychiatric and medical treatments and population health management quality and performance metrics.

  • Responsible for supervision of nursing staff and must have at least two years supervisory experience.

  • Demonstrate sound judgment and effective, solution focused, problem solving skills.

  • Ability to work effectively with diverse populations and cultural groups.
