Next Step Health Care MDS Coordinator/RN in Plymouth, Massachusetts
Plymouth Harborside Healthcare is seeking an experienced MDS Coordinator for a full-time position. The responsibilities of the MDS Coordinator includes but is not limited to overseeing and coordinating of developments and completions of the member assessment ( MDS / PPS) in accordance with current Federal and state regulations, coordinating and leading the development of a comprehensive member assessment and care plan for each customer as chairperson of the community's Interdisciplinary Plan of Care team, ensuring the face validity of all Minimum Data Sets before electronic submission and combine the timely electronic submission of all MDS, and conducting a monthly review of a random sample of MDS to ensure accuracy and internal consistency with the medical records. MDS Nurses are provided significant regional support from the industry's most experienced nurses and are provided a very great and respectable working environment.
Duties of the MDS Coordinator include but are not limited to:
Completes assessments, Minimum Data Set (MDS} and care plans for all residents assigned.
Monitors completion of MDS by other disciplines within timeframes prescribed by regulatory guidelines.
Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing.
Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
Initiates care plans and supporting activities that will result in best possible outcome for assigned residents.
Participates in morning clinical meeting
Conducts Utilization Review meeting. Ensures that all disciplines have updated and accurate information.Â
Participates in discharge planning, care plan meetings and family meetings
Maintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues as necessary.Â
Reviews all resident incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.
Responsibilities and requirements for MDS / PPS Nurse Coordinator:
Valid RN License in the state of MA
Clinical experience, education or specialty skills specific to geriatrics is desired.
Great interpersonal skills for effective daily interaction with Residents, family members and all members of the Interdisciplinary Team
Must have knowledge in PPS assessment as well as OBRA compliance.
Must be a self-starter and a high attention to accuracy and details and possess good time management skills
Ability to make independent decisions and follow through with limited supervision.
Must have at least 1 year of MDS coordinator experience. Additional experience and performance achievements preferred.
