Jordan Hospital LPN, Occupational Health in Plymouth, Massachusetts

Duties/Responsibilities:

· Is able to articulate Dept. philosophy and provide care accordingly

· Functions as a member of an interdisciplinary team that includes the patient and case worker in facilitating a patient specific plan of care

· Communicates with company clients and hospital managers employee's work status

· Provides skilled service that ensures the nursing needs of the patients are met as identified in the patient's initial and comprehensive assessments

· Obtains data on the physical factors that may influence the patient's medical surveillance.

· Maintains up-to-date documentation in the EMR so that problems, plans, actions and goals are accurately and clearly stated by all members of the team

· Responsible for maintenance, calibration as well use of clinical equipment

· Responsible for efficient and effective patient flow through clinic

· Responsible to ensure all reviews are conducted on Employee Health charts

· Conducts patient assessments for physicals, injuries, immunizations and return to work exams

· Conducts TB testing and fit testing as needed

· Documents in the electronic medical record per Dept. standards to ensure accurate retrieval of quality indicators

· Effectively navigates procurement process of appropriate medications through our pharmacy

· Proactively informs the Supervisor of unusual or potential problems

· Demonstrate knowledge of Dept. policies and OSHA Regulations

Education/Experience Required:

· Ability to read, write and speak English

· Ability to present in a self-assured, professional manner while functioning independently in a variety of settings

· Ability to sustain a positive attitude and connections to clients

· Ability to work effectively within an interdisciplinary team approach to care

· Minimum of 1-2 years of nursing experience within the past five years

· Demonstrated ability to manage all aspects of hospice terminal care, other than care planning and pronouncements

· Demonstrated ability to assess and respond to needs of patients and clients in varied settings and ability to communicate effectively with people from a variety of backgrounds

· Demonstrated customer service skills are essential

· Strong computer skills required

· Occupational Heath experience helpful

Registration/Certification:

· Current Massachusetts LPN license

· Valid Massachusetts driver's license and access to reliable transportation
