Jordan Hospital Addiction Nurse in Plymouth, Massachusetts


  • Conduct outreach visits as a member of a multidisciplinary team to engage individuals following discharge from the ED after an opioid overdose or individuals who are at risk for an overdose.

  • Offer linkages to treatment based on the individual’s need and readiness, including induction of Medication for Opioid Use Disorder (MOUD) via telehealth.

  • Once started on MOUD via telehealth, link individual to a community office-based addiction treatment (OBAT) through a warm hand off.

  • Utilize appropriate interventions to address current and/or future barriers to recovery, including transportation.

  • Provide consultation to other disciplines and staff on biopsychosocial, environmental and cultural factors.

  • Brief therapeutic intervention with patient and family around stigma, stages of change and harm reduction.

  • Formulation and implementation of support services to provide continuum of care for patients and families.

  • Assists in the development and/or periodic revision of clinical protocols for this community outreach model.

  • Provides ongoing education and proactive support of patients and coordination of services

  • Works with the pharmacy and the OBAT providers in managing medication related challenges.

  • Organizes random urine drug screens, as necessary

  • Completes prior authorizations, as necessary

  • Connects patients to detox or inpatient treatment assistance when needed in collaboration with behavioral health

  • Communicates with outside behavioral health facilities/providers and facilitates releases of information

  • Consistently uses clear concise and effective communication written/oral and comply with departmental policies when sharing/documenting relevant patient care data.

  • Completes appropriate documentation with excellent record keeping

Education/Experience Required:

  • At least two (2) year of experience working with patients with substance use disorders/addiction

  • At least one (1) year of experience with Medication for Opioid Use Disorder

  • Knowledge of and experience providing training on harm reduction strategies


  • Active Licensed Registered Nurse or Licensed Practical Nurse in the Commonwealth of Massachusetts
