Berkshire Health Systems Clinic Screener & Care Coordinator-Grant in Pittsfield, Massachusetts
Through case management assessments and reassessments, the Case Coordinator will work with clients to identify SMART goals, determine needs, develop action plans, and track progress towards self-sufficiency and an improved quality of life. • High school diploma required. College degree preferred. • Progressive two year experience in a clinical setting. • Knowledge of federal, state, local laws, regulations, and guidelines related to STIs, HIV, Hepatitis C, and TB. • Working knowledge of epidemiology, infectious diseases, and asepsis preferred. • Proven ability to participate in program planning and implementation including outreach. • Ability to assist the Epidemiologist RN with tracking, trending, & analyzing data to create action plans for performance improvement. • Excellent phlebotomy skills are necessary or must be obtained within the first 6 months in the position. • Prior experience working with individuals to assess their needs and assist in goal setting preferred. • Two years in an infection control capacity or working with clients with STIs, HIV, Hepatitis C, and TB preferred. Willingness to attain infection control, STI, Hepatitis C, TB knowledge required. • Excellent interpersonal skills with a variety of people including patients, families, employees, physicians, and agencies required. • Proven ability to exercise independent judgment and initiative.
POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
Prior experience working with individuals to assess their needs and assist in goal setting preferred.
Education and Training:
High school diploma required. College degree preferred. Must complete all mandatory trainings required by DPH and BMC.
