The Registered Nurse for the Adult Foster Care Program is a professional nurse who performs administrative duties in assessing the Adult Foster Care Member working as part of the multidisciplinary team.
Graduate of an approved school of professional nursing
Licensed registered nurse in the state of Massachusetts
Minimum of two (2) years experience in the direct care of the elderly or disabled adults
Salaried position up to $80,000 annually
Completes home visits to evaluate potential Members
Contacts the Member's Primary Care Physician to obtain medical information and physician clearance for the Adult Foster Care Program
Completes and signs the designated screening tool, nursing assessment forms; including the MDS-HC and coordinates all other applicable clinical assessments
Develops and reviews on an ongoing basis each Member's AFC Plan of Care
Monitor's the Member's health status and reports changes in the health status to the Member's physician
Provides routine home visits to Adult Foster Care members
Provides case management
Selects, trains, evaluates and supervises the AFC Caregiver in conjunction with the Care Manager
Provides ongoing training to AFC Caregivers on health and aging
Prepares the Plan of Care for the AFC Caregiver
Provides case management and participates in multidisciplinary team meetings on a regular and ongoing basis
Prepares the plan of care for the AFC caregiver and ensures implementation of the plan of care
Coordinates delivery of AFC services with any other health or supportive services
