BFAIR Day Habilitation RN/LPN Relief in North Adams, Massachusetts

POSITION DESCRIPTION: The Day Hab RN/LPN Relief is primarily responsible for providing various medical supports to individuals with disabilities in a Day Habilitation program. The RN/LPN provides medical treatment to individuals as needed and is responsible for recordkeeping and communicating with medical providers and advocating for the medical needs of the people we support. QUALIFICATIONS: Registered Nurse Degree or Licensed Practical Nursing degree required with at least one year of time involved in providing nursing supports in community services for individuals with developmental disabilities, as well as current license in the State of Massachusetts. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing). Valid US driver\'s license and ready access to a vehicle. TRAINING REQUIREMENTS: Receive and maintain current certification in First Aid, CPR, Safe Techniques to Avoid Restraint and Training (START), and BFAIR Human Rights Training. May need to attend workshops, seminars or BFAIR functions as needed for job development. RESPONSIBILITIES: 1. Treat each individual, co-worker, supervisor and others with respect and dignity due every human being. Adhere to the Department of Developmental Services (DDS) Human Rights Standards and the Disable Persons Protection Commission\'s (DPPC) mandated reporter guidelines around physical and emotional abuse, including the timely reporting of violations 2. Complete daily nursing notes as warranted, monthly nursing notes, quarterly health assessments, and monthly vital signs. 3. Work collaboratively with the Health Care Supervisor in the oversight of medication administration practices as per agency policy and governing regulations. 4. Monitor/order medical and infection control supplies through the Program Director. 5. Act as a liaison with the individuals served, parents and/or guardians, other service providers, physicians and/or medical resources, as well as between day and residential staff members, etc., to promote optimal programming and quality services. 6. Respond to all accidents, injuries and emergencies and recommend appropriate interventions such as ER visits, doctor\'s visits and going home. 7. Prepare any special reports as requested; meet weekly with Program Director and/or RN Supervisor. 8. Consult with hospital staff, family and or guardians, RN and other Program Directors as applicable on individual\'s medical concerns and review with program staff as warranted. 9. Maintain accurate and complete medical records, reports, and data necessary to meet the requirements of DMA, DDS, DPH, and BFAIR. 10. Adhere to agency/personnel policies, DMA, DDS, DPH regulations, follow emergency and call-in procedures, and maintain confidentiality.
