Recently named by The Boston Globe as one of The Top Places to Work in Massachusetts, our community based, mission-driven hospice and palliative care organization seeks a Per Diem Shift Nurse for Continuous Care to join its expanding hospice team. Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest.
Job Responsibilities:
This position provides direct patient care including on-going assessment, pain management. and symptom control in line with the plan of care established for the patient. The Shift Nurse notifies the Nursing Care Coordinator of any significant change in patient’s condition or any changes necessitating physician notification and documents all care provided including clinical observations and interventions.
Education, Skills and Experience:
Current Massachusetts LPN License in good standing.
Minimum of 2 years experience in community health, oncology nursing, or a related field preferred.
Comfort working independently in a one on one setting with patients.
