Join Our Team as a Licensed Practical Nurse Clinical Coordinator (LPN CC) at Elara Caring!

As a Hospice Licensed Practical Nurse Clinical Coordinator (LPN CC) , you will play a pivotal role in ensuring seamless patient care by supporting Clinical Team Managers (CTMs) in advancing workflows and overseeing patient journeys. You'll act as a key liaison between the patient, CTM, medical providers, and facility staff, ensuring the highest standards of care and communication. This will be a hybrid role to support leadership in the office but, be in the field will overflow and emergent needs.

Schedule: Monday through Friday from 8:00a to 5:00p

Your Role as a Lead LPN Clinical Coordinator:

  • Care Coordination: Monitor and ensure accuracy in Homecare Homebase (HCHB) documentation, ensuring all workflow requirements are met for seamless patient care.

  • Intermediary Communication: Act as the vital link between the patient, Clinical Team Managers, physicians, and facility staff to ensure smooth communication and care.

  • Patient Advocacy: Understand and respond to patient needs, manage their expectations, and advocate on their behalf to ensure the highest quality of care.

  • Decision-Making: Use your advanced problem-solving skills to make informed decisions, even in complex situations, and know when to escalate issues for resolution.

  • Conflict Resolution: Handle complaints, manage conflict, and negotiate positive outcomes for both patients and staff.

  • Leadership: Support your team in reaching their full potential by fostering a collaborative environment and helping them grow.

What You’ll Need:

  • Licensed Practical Nurse (LPN): Graduate of an accredited nursing school with a current LPN certification.

  • Experience: 1+ year of practical experience, with a preference for Home Health or Hospice settings.

  • Communication Skills: Ability to convey information clearly, listen actively, and respond with the appropriate tone and message.

  • Time Management: Advanced organizational skills to manage tasks and deadlines efficiently.

  • Tech-Savvy: Quickly become proficient in enterprise systems like Homecare Homebase and Workday, as well as Microsoft Office applications.

  • Patient-Centered Care: Ability to seek and respond to patient feedback, advocate for their needs, and manage their expectations.

  • Bilingual Skills (Preferred): Bilingual in English and Spanish is a plus.

  • Valid Driver’s License & Reliable Transportation : Required for travel to perform job tasks.
