Meeting Street Nurse Care Coordinator in New Bedford, Massachusetts

Nurse Care Coordinator Early Childhood Programs Meeting Street, a non-profit organization dedicated to providing services to children and families in Rhode Island and Massachusetts, is seeking a motivated team player to serve as the Nurse Care Coordinator in Welcome Families. As the Nurse Care Coordinator you will provide operational support to coordinate patient care and services, as well as promote and monitor effective and efficient utilization of resources. You will demonstrate clinical expertise in applying nursing processes, documenting skilled needs within regulatory guidelines, in matching community resources to the specific plan of care in collaboration with birthing hospital and community providers. You will function as a role model, collaborator, educator, consultant and change agent in the areas of continuity of care and utilization review. You will receive referrals from the birthing hospital, call families to schedule home visits, collect data using the Welcome Families forms and input into Welcome Families database. You will complete home visits and all components of assessments and data collection. You will give referrals/resources to families, conduct follow up call with all families and assess need for additional referrals/resources. You will send data to DPH for weekly analysis. You will attend meetings in community to identify resources for families. You will participate in Quality Assurance efforts to ensure timely and accurate data entry, participate in any site visits with DPH and participate in all training and mandatory meetings. You will collaborate with other Early Childhood programs for transition. You will join committees in the community to establish rapport. You will build community partnerships. Requirements The position requires that you are fully Bilingual in both English and Spanish languages. You are a Registered Nurse (BSN) Licensed in Massachusetts. You have experience for 2 years with maternity clients or newborn infants. You have experience promoting community-based programming. You are able to collect data and produce reports (EMR).
