*** We are offering a \$1,000 Sign on Bonus*** Family Service Association is looking for a caring, supportive Clinical Supervisor for our LTSS Community Partners program! Lead an interdisciplinary team helping children age 3 through adults age 64 with their long-term care needs in our Mashpee branch of our 4 LTSS Community Partners program sites (New Bedford, Fall River, Brockton, Mashpee). About the Program: Clinical Care Manager in our Long-Term Support Service Community Program (LTSS CP). The Clinical Care Manager (CCM) serves as the supervisor of the LTSS CP Supports Care Coordination team. The CCM provides regular supervision for Care Coordinators & Community Support Workers & the delivery of all LTSS CP Support Interventions. Responsibilities: Supervise Care Coordinators & Community Support Workers on a regular (no less than bi-weekly) basis using in group & individual settings. Demonstrate knowledge & experience in the delivery of LTSS Supports & Care Coordination. Ensure Care Coordinators develop person-centered care plans that reflect the voice & choice of assigned enrollees within required time frames. Ensure LTSS CP Supports care plans are endorsed by primary care providers & shared with all members of the enrollee?s care team. Demonstrate willingness to work with individuals with complex LTSS & BH needs. Support the engagement of assigned enrollees within specified timeframes. Promote achievement of Quality Measures for the LTSS CP Supports. Support the development of Consumer Advisory Boards & provider networks. Have a willingness & ability to work with ACOs, MCOs, state agencies, service providers & community organizations. Willingness & ability to work flexible hours & across the geographic area served by the program. Provide guidance to the care coordination team for the development of health & wellness coaching & current listing of community resources. Participate in 24 hour on call coverage on a rotating basis. Triage referrals & provide referral resource with accurate & timely availability of needed resources. Maintain paperwork requirements for LTSS CP Supports in compliance with established timeframes. Assist in the orientation of new employees & the ongoing professional development of all LTSS CP Supports employees. Provide assistance to the Director in reporting & monitoring program performance. Demonstrate & promote respectful, culturally competent & linguistically responsive delivery of services. Understand & demonstrate commitment to agency mission, values & philosophy in performing job responsibilities, including compliance with all ethical & legal requirements of the organization. Respond to reasonable expectations of the Director of Healthcare Transformation & the organization. Experience: Must have a minimum of 1 year experience as a supervisor, & a minimum of three years of relevant professional experience. Must have the skills necessary to engage & work with individuals from diverse backgrounds. Maintain a nonjudgmental attitude toward families. Must be willing to work across the geographic region assigned to the team & demonstrate willingness to work with the target population. Care Coordination, Bi-lingual capacity is desired. Candidate must have a Master\'s Degree or a RN with behavioral health expertise. LCSW, LICSW, LMHC, LMFT, LADC 1, Licensed Psychologist, Licensed Rehabilitation Counselor, Registered Occupation Therapist
