Family Service Association - Fall River Clinical Care Manager - Cape Cod in Mashpee, Massachusetts

Family Service Association is seeking a full time (40 hours per week), Monday through Friday, 8:30 a.m. - 5:30 p.m.   Position may be partially remote with drop in office in Mashpee, and includes travel in surrounding Southern Mass. area.

Duties and Responsibilities:

  • Provide direct management and supervision to assigned CP Team, consisting of Registered Nurse(s) (RNs) and Care Coordinators (CCs), using group and individual settings.

  • Provide training and coaching to CP Team via individual and group interventions.

  • Ensure CCs/RNs develop person-centered Comprehensive Assessments and Care Plans that reflect the voice and choice of assigned Enrollee’s including care needs, functional needs, accessibility needs, goals, and other characteristics within required CP timeframes.

  • Responsible for providing final review and sign-off approval of the Enrollee’s Comprehensive Assessment and Care Plan. Responsible for documentation of Care Plan Complete note in the Electronic Health Record.

  • Ensure the Comprehensive Assessment and C are P lan are shared with the Enrollee’s Primary Care Provider , all members of the Enrollee’s Care Team, and other providers who serve the Enrollee in compliance with contractual requirements, as agreed upon by the Enrollee.

  • Support the engagement of assigned Enrollees within specified timeframes.

  • Provide case consultation and/or attendance at Integrated Case Conferences for members with complex medical and/or social determinants of health (SDOH) needs.

  • Work collaboratively with and serve as a resource for A COs / MCOs, Health Centers, PCP practices, including in regularly scheduled meetings.

  • Work to collaborate with state agencies, Social Service Organizations, and other community partners, including participation in an area provider committee.   

  • Promote achievement of Quality Measures as determined by EOHHS, including p roviding assistance to the Director in reporting and monitoring program performance.  

  • Demonstrate willingness to work with Enrollees with complex medical and LTSS, Behavioral Health, Homelessness, Substance Use Disorder needs, as well as Enrollees involved in the legal system.

  • Ensure culturally and linguistically appropriate health care coordination services that are respectful of and responsive to cultural and linguistic needs of Enrollees. 

  • Advocate for and assist with navigating any Health Inequities discovered.

  • Provide logistical and clinical guidance and expertise to the C are Team for the delivery of LTSS CP supports. 

  • Utilize the specific expertise of Affiliated Partners for case consultation, care planning, transitions of care, and training development and deployment.

  • Demonstrate knowledge and experience in the delivery of LTSS CP supports and care coordination, including Health and Wellness coaching, support for transitions of care, etc.  

  • Ensure appropriate use of program’s EHR system for fidelity and accuracy of timely reporting of all CP Team members.

  • Support the development of the LTSS CP Consumer Advisory Committee (CAC) and provider networks.    

  • Demonstrate willingness to work flexible hours, and travel across the geographic area served by the LTSS CP Team, with occasional travel outside of that geographical region.

  • Assist in the orientation of new employees and the ongoing professional development of all LTSS CP Team employees.   

  •  Demonstrate and promote cultural competency awareness and sensitivity to race, religion, ethnicity,   disability, gender identity/gender expression, sexual orientation, geography, language etc.

  • Understand and demonstrate commitment to agency mission, values , and philosophy in performing job responsibilities, including compliance with all ethical and legal requirements of the organization.

  • Work in a professional and confidential capacity related to all program services and communications.

  • Work independently as well as part of a multidisciplinary Care Team.

  • Triage and balance competing priorities.

  • Participation in ongoing CQI activities.

  • Attend regular supervisory meetings both on a team and individual basis, as required.

  • Respond to reasonable expectations of the Agency and/or supervisor .   

Education and Experience Required:

  • Must have at a minimum at least two years of relevant professional experience of which at least one year experience as a supervisor. 

  • Must hold one of the following qualifications: 

  • RN with Behavioral Health expertise

  • LICSW

  • LMHC

  • LMFT

  • LADC I

  • LCSW

  • Licensed Psychologist

  • Licensed Rehabilitation Counselor

  • Registered Occupational Therapist. 

  • All licensures and registrations must be in good standing with the Commonwealth of Massachusetts. 

    Experience Desired:  Care Coordination, BIPOC and Bi-lingual candidates encouraged.
