Connected Home Care Home Health Aide in Malden, Massachusetts

DESCRIPTION: The purpose of the Home Health Aide is to assist consumers in maintaining maximum independence in their home environment. Home Health Aides provide hands-on personal care, simple procedures as an extension of nursing or therapy services as delegated by the nurse or therapist, assistance in ambulation or exercises, and assistance in medication reminders that are ordinarily self-administered. RESPONSIBILITIES: (includes but not limited to) Assist clients with personal hygiene, including shower, tub or bed baths, oral care, hair and skin care. Assist clients in the use of toilet facilities, including bed pans. Assist clients with transfer, including the use of mechanical lifting equipment as reviewed and documented by staff development and/or nursing supervisor. Assist clients with walking, including the use of walkers and wheelchairs, when applicable. Meal preparation and feeding, when required. Record and report changes in physical condition, behavior or appearance to supervisor. Documenting service delivered in accordance with Connected Home Care policies and procedures. (Completing Treatment Logs and Daily Visit Reports) Always Ensure client?s safety by supervising closely and being SAFETY CONSCIOUS at all times. Housekeeping; including washing floors, dusting, vacuuming, cleaning rooms, laundry. Shopping/errands, escorting clients to doctor appointments Read and follow the written care plan. Medication reminders. Participates on care team by providing input and making suggestions. Carry out exercise programs, in accordance with the care plan reviewed by nurse supervisor. Participates in care team meetings by providing input and making suggestions REQUIREMENTS Ability to participate in physical activity. Ability to work for extended periods while standing and being involved in physical activity. Ability to lift 50 lbs. Frequent Local Travel Ability to do extensive bending, lifting and standing on a regular basis. Attend in-service training sessions and staff meetings as directed by staff. QUALIFICATIONS: Have a sympathetic attitude toward the care of the sick and elderly. Ability to carry out directions, read and write. Maturity and ability to deal effectively with the demands of the job. Must have a current Home Health Aide certificate. Must have one (1) or more years? experience as HHA (Preferred) . Must have reliable Transportation
