The Home Care Case Manager (CM) has daily responsibility for developing comprehensive person-centered service plans and authorizing, coordinating, and monitoring community-based services to an assigned number of consumers. You will have your own case load and visit with consumers' in their home.

Qualifications: •Minimum of a Bachelor's Degree in Human Services or 50% of Bachelor's Degree completed with degree obtained within three (3) years of employment.•Minimum of one (1) year experience in Human Services.•Bilingual is preferred.•Proficient in the use of computer systems.•Must have reliable transportation and valid driver's license.•Excellent customer skills and professional demeanor with the ability to handle confidential situations.

Hours: Full Time – 40 hours per week (Monday-Friday)
