Greater Lynn Senior Services provides a wide range of social services for elders and people with disabilities -- information, transportation, meals, advocacy, home care, housing, and more--for one reason: to help those 60 and older in Lynn, Lynnfield, Nahant, Saugus and Swampscott live fuller, more independent lives, safely and with dignity. Duties:

GAFC and AFC RN will conduct onsite assessments for Group Adult Foster Care, Adult Foster Care and SCO Consumers according to the program guidelines. This RN will develop personal care plans according to the program's guidelines and will determine consumer clinical eligibility for Medicaid funded services in the community. For the AFC and GAFC programs, the RN co-manages AFC cases with the AFC and GAFC Case Managers. For AFC consumers, the RN will provide nursing coverage in compliance with 130 CMR 408.433(B) (2). For GAFC consumers, the RN will provide nursing coverage in compliance with GAFC program rules and regulations.

Qualifications:

Bachelor of Science degree in Nursing or Associates of Science degree in Nursing or Diploma RN and at least two (2) years of community-based nursing experience or long term care direct service experience preferred. Two years of recent experience in the direct care of the elderly or disabled adults is required. RN must be fully licensed by the Massachusetts Board of Registration in Nursing. Valid Massachusetts Driver's License to make home visits is required. Strong organizational and communication skills are a must. Applicants must have the ability to work independently and with an interdisciplinary team. Strong clinical assessment skills and strong computer skills are required.Hours:

Part Time, 20 hours per week
