Tufts Medicine Registered Nurse, RN, ED Aftercare Clinician in Lowell, Massachusetts

Job Summary:

The ED aftercare clinician reviews all of the final laboratory and radiology test results on discharged ED patients to ensure they have the appropriate discharge plan. Works collaboratively within department and other services to provide complete connected care to patients. Acts as a liaison between the patient and the healthcare system to ensure the best possible care.

Key Responsibilities and Duties:

  • Reviews all final laboratory and radiology reports on discharged patients from the Emergency Department.

  • Takes action if finding noted in the Emergency Department record are inconsistent with the final results, to ensure appropriate follow up for the patient. Consults with the ED provider or patient’s primary care physician for any questions and/or further orders for the patient.

  • Documents interventions in the follow up portion of the Emergency Department medical record.

  • Completes any necessary mandatory reporting paperwork and documents within the medical record.

  • Places follow up phone calls to patients as able to ensure follow up as instructed, and also improve the overall patient experience. Refer patients to PCP or follow up in the ED if any concerns.

  • Calls patients in follow up to ED visit if referred by ED staff. This may be an at risk patient or someone that the ED staff is requesting follow up nurse to speak with regarding their discharge plan.


  • Current MA license and BSN is strongly preferred

  • Valid BLS required

  • 3+ years of RN experience, critical care or ED preferred
