Wellforce Manager of Patient Flow in Lowell, Massachusetts

This is a great opportunity to join a 3-time Magnet recognized community hospital. We are seeking an experienced Manager with patient flow experience to work in our Operations Center where you will be responsible for optimizing patient flow through our hospital and across our health system.

The Manager of Patient Flow oversees the day to day patient flow throughout the organization. Works collaboratively with key stakeholders to optimize patient flow throughout Lowell General Hospital.

As Manager of Patient Flow , creates an environment that enables the hospital to fulfill its mission of patients first in everything we do and meet or exceed its goals. The Manager is accountable for overseeing flow coordination including but not limited to admissions, transfers and discharges and all patient movement throughout the organization. Will work collaboratively with key stakeholders to optimize resources needed to support patient flow, care transition and positive patient outcomes. Exhibits excellent verbal and written skills for high level communication within the hospital and at interdisciplinary and administrative meetings.

  • Creates a healthy work environment that focuses on the patient and employee experience.

  • Leads through change and demonstrates effective leadership skills of delegation, organization and coordination in directing patient flow.

  • Maintains an inclusive workplace, maximizes contributions of all stakeholders to enhance patient placement.

  • Works in collaboration with the bed facilitator, ED and admitting providers to optimize patient flow and throughput.

  • Communicates with community stakeholders such as Primary Care Providers, PHO, Home Health Foundation, Chronic Care Clinic and others to create a seamless admission and discharge to and from Lowell General.

  • Collaborates with RN Navigator and Case Management to assist in flow of complex patients.

  • Communicates with Saints and Main campus to achieve campus balance.

  • Demonstrates ability to collect, track and trend data in order to identify opportunities for performance improvement and improve patient outcomes.

  • Active lead in Epic implementation and execution of Operations Center dashboards.

  • Attends bed meetings to facilitate optimal patient flow.

  • Will track stats and metrics for dashboard reporting.

    Hours: Full time, 40 hours per week.

    Requirements:

  • MA RN License is required

  • BSN strongly preferred

  • Prior experience in hospital patient flow in a patient transfer center, 3-5 years required

  • Strong communication skills, and ability to work collaboratively with multiple stakeholders
