Wellforce Director of Case Management in Lowell, Massachusetts
Circle Health / Lowell General Hospital is seeking a strategic leader in the role of Director of Case Management. This role reports to the Associate Chief Nursing Officer and is responsible for case management and social work staff and processes including inpatient clinical areas and the Emergency Department.
The Director assures that systems and infra-structure are in place to facilitate entry of acute care patients to the appropriate facility and unit for all bedded patients. In this role you will serve as an expert in utilization review and management and you are responsible for managing and enhancing relationships and services in the community.
The daily responsibilities include:
Studies, evaluates, selects and implements systems to facilitate entry of acute care patients to the appropriate facility. Once implemented, evaluates effectiveness and leads improvement efforts system wide to provide an effective admission structure.
Develops and implements strategies with internal and external providers to ensure we are able to place patients in the right level of care at the right time predictably. Works closely to outline priorities and approval processes.
Directs the overall operation of the Social Service Department in accordance with federal, state, and local standards, guidelines and regulations that govern our facility to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.”
Develops strong relationships and partners with external post acute providers
Studies and implements the case management model and structure to achieve sustaining appropriate length of stay resource utilization and clinical quality care for patients over time. Responsible for designing and implementing improvement strategies for the care model on an ongoing basis.
Oversees all activities related to referral management. Develops collaborative relationships with payers and maintains compliance with payer contracts for utilization management.
Oversees the psychiatric nurse practitioners who have oversight of inpatient population.
Leads and direct the organization to implement, and sustain over time, predictable plans for patients either pre or on admit to manage appropriate admissions and lengths of stay.
Resources and educates staff and the organization on an ongoing basis national and regional trends and adjusts case management model to meet financial, regulatory and quality goals.
Develops annual budget for areas of responsibility and works with department leaders and staff to implement the budget and assures measures for staying within budgetary parameters.
Leads development and implementation of strategies related to management of longitudinal care for patients connecting the pre and post acute care with the short term acute care setting.
Works closely with Nurse Executives to develop a 5 year plan for areas of responsibility including implementation plans, timeline projections, financial projections, etc.
Plans and implements recruitment and retention strategies for areas of responsibility in collaboration with Human Resources.
Builds and maintains strong working relationships with colleagues internally in order to achieve throughput, length of stay and clinical quality goals.
Develops, monitors and reports metrics to monitor effectiveness of utilization management and discharge planning strategies and effectiveness of all areas of accountability
Current MA RN License
Master's Degree Required
Minimum 5 years of experience, including 4+ years of management experience
Case Management Certification Preferred
