Position Summary: The ED Case Manager is the central point of contact to ensure patients are cared for in the right care location which includes acute, ambulatory, home care, skilled nursing facility, mobile integrated health, or Circle's outpatient ConnectCare Hub. In this role you will:
Serves as a patient advocate
Proactively uses automated data algorithm to identify all patients at high or emerging risk for readmission upon arrival to ED.
Assesses high or emerging at risk patients' specific physical, emotional, educational, social, and cultural care needs and uses assessments as a basis for developing a Plan of Care that addresses gaps in care.
Collaborates with all members of the interdisciplinary team to facilitate patient flow to right location of care that supports reducing gaps in care.
Collaborates with physicians, healthcare teams, and the patient/family in determining patient’s Plan of Care.
Implements the Plan of Care to achieve the identified expected outcomes for the patient.
Utilizes home care, skilled nursing facilities, mobile integrated health, Circle's ConnectCare Hub, and other community resources and systems to implement the plan of care by facilitating referrals when appropriate to reduce physical, social, and/or economic barriers to care.
Provides education for the patients and families and documents according to policy.
Prepares, organizes, and presents data on patient outcomes which include volumes and care location outcomes.
Substantiates activity by documentation in the patient record that is entered in a clear, concise, organized, and timely manner.
Assists in meeting population health goals by linking families and communities to essential resources.
Hours: Every Saturday and Sunday, 7am-3:30pm.
Location: Main Campus
MA licensed RN with BSN preferred.
2+ years of clinical experience
Some discharge planning or case management experience preferred.
Requires strong written and verbal communication skills.
