Your skill and experience can make a difference in the lives of people nearing the end of their life’s journey.
As a Chaplain, you are a member of the hospice team with advanced training in theology and/or psychology who is able to provide a wide range of supportive services to people from a variety of religious backgrounds and experiences during a time of crisis in their lives.
Provides spiritual support to patients and families, contact all patients/families by phone within 3 days and completes a Spiritual Assessment Form within 5 calendar days of admission.
Based on assessment and patient /family wishes, provides spiritual support in a nonjudgmental manner.
Communicate patient/family reports concisely and effectively to team.
Participates in family conferences and serve as a liaison with the patient’s spiritual community leader.
Assists with funeral and memorial services when requested and collaborate effectively with other members of IDT.
Assists with hospice education and memorial services in community and participate in achieving MVH goals and objectives.
Master’s degree in religion/theology, pastoral care, human services, or counseling with at least one unit of CPE (Clinical Pastoral Education)
Some field experience in ministry.
Demonstrates knowledge of and sensitivity to, the spiritual issues expressed by the terminally ill and their families.
Demonstrates knowledge of the grieving process.
Demonstrates knowledge of the Hospice philosophy of health care delivery.
Ability to be open, flexible and ecumenical in ministry.
Assisting in servicing Southern New Hampshire and Northwestern Massachusetts
