Family Service Association of Fall River RN Case Manager in Hyannis, Massachusetts

Position: Registered Nurse / Case Manager Program: Adult Family Care FLSA Status: Non-Exempt WCC Code: (8835) Accountability: Directly responsible to the RN Supervisor _________________________________________________________________________________________ Job Summary: The AFC Registered Nurse/Case Manager evaluates potential members and foster families, monitors member/caregiver placement status, provides case management services, serves as liaison with assigned member?s primary care physicians, monitors member?s health status, completes semi-annual health status reports and member care plans, implements and evaluates caregiver training sessions and assists in outreach to increase community awareness of AFC Program services. Duties and Responsibilities: 1. Evaluate potential members and caregivers. 2. Serve as an interdisciplinary team member in the matching and placement process. 3. Monitor placement status according to department guidelines. 4. Serve as liaison with primary care physicians for medical information and service authorization of potential members. 5. Coordinate and provide member and caregiver family counseling. 6. Complete member medical assessment forms. 7. Monitor assigned member?s health status. 8. Review and member?s personal care services and ADL logs at least every other month during home visits. 9. Complete semi-annual health status reports and multidisciplinary care plans. 10. Provide case management services. 11. Write progress notes and maintain member/caregiver records in a timely fashion. 12. Plan, implement and evaluate caregiver training sessions. 13. Assist in outreach efforts through regular public speaking engagements and meetings with area professionals in an effort to increase community awareness of program services. 14. Attend weekly supervisory meetings both on a team and on an individual basis, as required by the Program Director. 15. Assist in the delivery of other required program services, as necessary. 16. Respond to reasonable expectations of the Agency and/or supervisor. Education and Experience: The AFC Nurse will be a registered Nurse. Recent (within 2 years) experience with geriatric and disabled adult populations in a community setting preferred. Must be currently MA licensed in good standing and CPR certified.
