Cape Abilities Health Care Supervisor - RN in Hyannis, Massachusetts

GENERAL SUMMARY Provides direct care & coordination of medical services to program participants. ESSENTIAL JOB FUNCTIONS* Provides direct care, as needed. Provides training in relevant health topics for staff & clients, as needed. Coordinates medical services with each participant?s primary physician or medical clinic. Oversees all health care services provided to program participants. Participates in the ongoing interdisciplinary evaluation & review of each participant?s needs & status. Obtains reports & approval of treatment plans from primary physicians. Documents recommendation in participants? records resulting from periodic reviews. Advises program management & staff of any medical problems that may hinder a participant?s progress or that may be negatively impacted by program activities. Completes, signs &/or reviews monthly progress notes in accordance with regulations established by the Commonwealth of Massachusetts-Executive Office of Health & Human Services (EOHHS). Serves as mandated reporter for suspected cases of abuse &/or neglect to the Disabled Persons Protection Commission (DPPC). OTHER DUTIES & RESPONSIBILITIES Performs additional duties, as assigned. PREPARATION, KNOWLEDGE, SKILLS & ABILITIES Registered Nurse (RN) licensed by the Massachusetts Board of Registration in Nursing. Minimum 1-year prior experience in a community health setting. Must be able to effectively communicate in both verbal & written form with coworkers & supported individuals & read fluently. Valid driver?s license in the state of Massachusetts required.
